Successful autologous stem cell transplantation (ASCT) in a patient with juvenile dermatomyositis

چکیده
We report a 16 year old patient, who was diagnosed with JDM four years ago with severe muscle weakness and skin involvement. Despite therapy including methotrexate, steroids, immunoglobulins, cyclosporine A and rituximab a sustained remission could not be achieved. The patient developed progressive contractures due to persistent inflammatory reactions. Due to treatment-refractory disease immunoablation followed by an autologous stem cell transplantation (ASCT) was performed. The stem cells were mobilized by application of 2 g/m2 cyclophosphamide. After immunoablation using ATG (thymoglobuline®) 10 mg/kg, cyclophosphamide 120 mg/kg and fludarabine 150 mg/m2, CD3/CD19-depleted CD34+stem cells (7.5 × 106/kg) including 2,9 × 104/kg Tcells were transferred. The haematological reconstitution with leucocytes >1000/ml and granulocytes >500/μl was achieved on day +7 and day +8, respectively. Substitution of erythrocytes or platelets was not necessary. No severe infections or organ toxicity (WHO) were observed.

Oral Ciprofloxacin Prophylaxis in Patients Undergoing High Dose Therapy and Autologous Hematopoietic Stem Cell Transplantation
Background: Antibiotic prophylaxis is usually used in allogenic stem cell transplantation, but its use in Autologous Stem Cell Transplantation (ASCT) is controversial. We evaluated the efficacy of ciprofloxacin prophylaxis in ASCT. Materials and Methods: To identify the efficacy of ciprofloxacin on the incidence of neutropenic fever and its complications, 72 patients had been admitted to Talegh...

a change in proinflammatory plasma cytokine profile of patients with juvenile dermatomyositis correlating with disease activity Juvenile dermatomyositis ( JDM) is a rare autoimmune disorder, affecting mainly muscles and skin. The mainstay of treatment is high dose corticosteroids, combined with other immunosuppressive drugs. In about 30% of patients, the disease cannot be controlled despite mul...

متن کاملSuccessful Treatment of Low-Dose Lenalidomide Maintenance Therapy Followed by Second Autologous Peripheral Blood Stem Cell Transplantation in Heavily Treated Multiple Myeloma
Recently, the prognosis of multiple myeloma has been improved by using high-dose chemotherapy followed by autologous peripheral blood stem cell transplantation (ASCT), bortezomib, and immunomodulatory drugs including thalidomide and lenalidomide. On the other hand, treatment strategy remains difficult for refractory and relapse cases. Here, we report the successful treatment of low-dose lenalid...
متن کاملAutologous stem cell transplantation in autoimmune arthritis restores immune homeostasis by renewal of the natural Treg compartment
Autologous stem cell transplantation (ASCT) is a last resort treatment for refractory juvenile idiopathic arthritis (JIA). It has been shown to induce dramatic and long-term improvements, but the underlying mechanisms remain to be elucidated. We investigated the potential role of regulatory T cells (Treg) in the processes of immune reconstitution and re-establishment of immune tolerance followi...
